Frequently Asked Questions

  • A Good Faith Estimate (GFE) form is used by private practices or clinics to provide their best judgment of the estimated cost of care based on a patient’s projected plan of care. It is required by the No Surprises Act (P.L. 116-260) for providers with patients who are either not insured (i.e., self-paying) or not choosing to bill their insurance plan directly (i.e., considered out-of-network). The estimate can include a range in terms of numbers of visits and codes billed since a plan of care may change based on several factors, including patient progress, patient adherence to treatment, or other comorbidities that a patient may experience. Below are costs associated with each session type offered.

    The length of the session includes all direct interventions, caregiver coaching, progress check-ins, etc. If a child is unable to attend to direct activities for the duration of the session, skilled caregiver coaching will be conducted.

    • 45-Minute Mobile Session: $90 + $30 travel fee for first 15 miles from Lake Butler, FL (32054). Greater distances will be calculated on a case-by-case basis. Please contact us for pricing. 

    Virtual Treatment Sessions:

    • 45-Minute: $90

    30-Minute Virtual Caregiver Coaching Session: $60*

    *not able to be reimbursed through SuperBills nor paid with HSA.

    Initial Evaluation: Depending on the complexity/needs of the evaluation, costs can range from $250 - $350. Evaluations can last 1-2 hours and you will be provided with a written report following the finalization of results.

    Evaluations may not be necessary due to recent evaluations completed at school or with another practice. This will be determined on a case-by-case basis.

    Group sessions with daycare, homeschool group, or other community events are priced on a case-by-case basis. Please inquire for more information.

  • Clients have the option to enroll in a subscription model for services to decrease the cost per session by 10%. Depending on a child’s treatment plan, you can pay for the sessions monthly and schedule sessions based on your availability.

    For example, if a client has a treatment plan of 1 session weekly, a subscription payment would include x4 sessions/month for $324 rather than $360. It is the client’s responsibility to schedule themselves for the month. Subscription clients are entitled to short-notice cancellations (2 hours before) without cancellation fee being applied. They are responsible for rescheduling their visit for the billing period.

    Subscriptions cannot be submitted to insurance via a SuperBill. They can be paid via HSA card.

  • We offer mobile speech therapy sessions within a 20 mile radius from Lake Butler, Florida. Services at a wider radius may be scheduled at an additional rate commesurate with the distance. For more information, please submit an inquiry via the contact page.

    Mobile services allows for the ability to target goals in natural environments including homes, daycares, ABA clinics, private schools, or any other location that is part of your day-to-day life! Depending on the location, it may require prior approval by the site to allow the speech therapist to enter. This is especially a consideration for schools and ABA clinics.

  • Yes! Virtual sessions, or ‘teletherapy’ are offered to clients across Florida and Georgia. This can be perfect for clients who are not local to Lake Butler, FL or for clients needing more flexibility. I have served children virtually for two years - it is equally effective compared to in-person services when the client is appropriate.

  • We provide services in the areas of articulation disorders, phonological process disorders, motor speech disorders, fluency, voice, early language intervention, elementary/middle school language intervention, literacy (using the UFLI program), and executive functioning.

    We offer evaluations and 45-minute sessions either virtually or in-home/community locations.

  • We offer a free, no obligation, 15 minute consultation to discuss your concerns and explore if speech or language therapy could be indicated.

  • At this time, we are not considered ‘In-Network’ with any insurances and are strictly private pay, meaning clients are responsible for payment. As this changes we will update this page.

    We can provide you (the client) with a Super Bill. This is a document that outlines the services rendered and cost. You may submit this to your insurance for reimbursement, though the amount varies by insurance and plan. This can depend on your plan’s co-pays, co-insurances, deductible, and visit limitations. To understand more, contact your insurance to discuss out-of-network benefits for speech therapy services. This is a responsibility of the client.

    We are credentialed to accept the Step Up For Students scholarship through FES-UA (Family Empowerment Scholarship for Unique Abilities) funds.

  • We accept debit/credit card, HSA card, check, and cash. Upon completing your intake at the evaluation you will be asked to keep a credit card on file at all times to aid staff in ensuring timely payment.

  • We love working with caregivers who are eager to learn and support children throughout their development. There is a breadth of evidence indicating caregiver coaching as one of the most crucial components in progress towards developmental milestones. Expect during sessions to learn home strategies and be given home practice recommendations to implement between sessions.

  • Yes! We can find a time/location that aligns with a homeschooled student’s schedule easily. If you are interested in group homeschool speech/language enrichment sessions at a discounted rate per student, please contact us as this is discussed on a case-by-case basis.

  • In most cases, the answer is no, because we are a self pray practice a referral is not necessary.

    However, if you would like to submit superbills to your insurance carrier, you can request a referral to Evergreen Speech and Language Therapy from your PCP prior to scheduling an evaluation. This may need to be provided at a later date for reimbursement depending on the carrier.
